About the Family Medical Centerin ElDorado
The UAMS El Dorado Family Medicine Center provides primary care medical services to patients of all ages. We cover all stages of life from pediatric conditions to senior health care.
The clinic provides convenient diagnostic and support services on site. We are also able to perform minor office surgical procedures in the clinic. We offer routine adult and pediatric immunizations
Prescription Information
To refill a prescription, please ask your pharmacy to send an eScribe request to our clinic, or you can request a refill during your appointment. If you originally filled your prescription at the UAMS Health Outpatient Pharmacy, you can also refill your prescription online using MyChart.

Free parking is available in a parking lot next to the building.
Directions From the Parking Area
Enter through the main entrance (a covered entrance) which is on the west side (the side facing N. Thompson Ave.) Once inside, walk straight to the the elevators (or stairs to the left of the elevators) and go the to the second floor. Once you are on the second floor,
From the elevator to the clinic: After getting off of the elevator on the second floor, turn right to enter the patient waiting area for the UAMS Health Family Medical Center. The check-in desk is on the far side of the waiting room.
From the stairwell to the clinic: After leaving the stairwell on the second floor, turn left to enter the patient waiting area for the UAMS Health Family Medical Center. The check-in desk is on the far side of the waiting room.

Appointment Information
What to Bring to Your Appointment
Their photo ID, their insurance card, a list of any medications they’re taking, any medical records they can provide (including slides or test results from other clinics)
What to Expect at Your Appointment
For your first appointment, please check in 15 minutes before your appointment time. We will need to make sure we have your correct contact information, date of birth, address, a working phone number, and email, if you have one. We will need to verify your insurance. All co-payments and deductibles are due when you check in for your appointment.
Your first visit will be your longest visit, as we will need to enter your information into our Electronic Medical Record. This visit will include a physical exam and we will go over your medical history. Please bring with you all medicines that you take (in their original bottles), including prescriptions, over the counter, vitamins, and herbal supplements.
Telemedicine Information
In an effort to keep our patients and health care workers safe, we are offering virtual visits and phone visits.

UAMSHealth MyChart
UAMS Health MyChart is a free online patient portal that gives you free, secure 24-hour access to your health information. You can see your lab and test results, send messages to your doctor, make appointments, pre-register for appointments, refill prescriptions and pay your bills. Ask your provider or the clinic staff to send you a sign-up link by email or text. Once you have this link, you have eight hours to sign up with just your name and date of birth.
